Plastic surgery is a mechanism that allows a Sim to change their facial and/or body structure. This feature appears in The Sims 2: University and The Sims 3: Pets.

The Sims 2: University[edit | edit source]

Sims who work in the Show Business career will get Dr. Vu's Automated Cosmetic Surgeon, which is unlocked at level 6. When this career reward is used successfully, the face-shaping controls from the Hair and Face section of CAS will appear, allowing the player to change the Sim's facial structure. The Automated Cosmetic Surgeon does not have an Undo option, so changes made by selecting any of the pre-made templates can only be reversed by canceling the plastic surgery. Once the Sim's face has been modified, the effects of plastic surgery cannot be undone, but there is no limit to the number of times a Sim can use the Automated Cosmetic Surgeon. Only young adults and older can use this object, but there is a mod that will allow teens to use it.

Each time a Sim uses the Automated Cosmetic Surgeon, there is a chance that the machine will malfunction and change the Sim's face in random ways, causing him or her to have a twisted, distorted face. While the Sim's face cannot be untwisted, successful use of the Automated Cosmetic Surgeon will allow the player to give the Sim a normal face by selecting one of the pre-made face templates.

Plastic surgery is purely cosmetic, and does not affect Sims' genetics. Thus, a child from a Sim who has used the machine will carry that Sim's original genetics. Also, if the Sim uses a broken face template, plastic surgery will not affect that.

The Sims 3: Pets[edit | edit source]

Sims can get plastic surgery from the hospital. Sims can get either face surgery to change their face structure, or body surgery to change their weight, muscle mass, muscle definition, and breast size, respectively. There are 6 options for the plastic surgery:

  • Cheap facial beautification (§950)
  • Expensive facial beautification (§2,800)
  • Corrective face surgery (§2,050)
  • Cheap body modification (§1,100)
  • Expensive body modification (§3,250)
  • Corrective body surgery (§2,275)

If the Sim goes through cheap plastic surgery, there is a 40% chance that the surgery could go wrong and they will come out with a different face or body shape that the player originally intended them to have. This, however, can be fixed for an additional sum. The chance of failed plastic surgery is lowered to 10% if the player chooses the expensive plastic surgery options.

Sims that are currently wearing a career outfit cannot get plastic surgery. If the player attempts to give them one, they will be told that the Sim has decided not to undergo surgery.

Aliens[TS3:S] cannot receive plastic surgery.
