I have fixed your userbox and it's title. I've moved it to Template:Userbox potatoes. However, you should know a few basics before creating them:
The syntax of the code should be vertical instead of horizontical. This makes the template much easier to edit and makes sure you don't miss anything.
Things like "Original idea by" never go into templates, nor any page into the wiki.
Several tags to categorize the userbox and it's users are used. You can see them by editing the template's page.
I'm, however, a bit concerned about how appropriate the template is, as it does not fit the wiki's theme. It think it would best fit in [www.uncyclopedia.wikia.com Uncyclopedia].
