Some common stories about famous NPCs who died due to Death By Flies all seem to not have been confirmed by EA games and any other third party OR internal sources, one of the most famous stories is the theory about Bella Goth.

The Bella Goth Fly Story:

A less widely known story is the theory that the reason Bella Goth is "missing" is because she was eaten alive by flies and that her grave is hidden somewhere in the Goth Manor in The Sims 2. Although it is contradicted in The Sims 2 in the neighborhood Strangetown, many people still seem to think that it is true. The originis of this story are unkown but thought tho have started on a fan site.

Being eaten alive by flies is a form of death in The Sims 2. The event can be triggered if there are many (10-15) dirty, fly infested objects emitting green smoke in a room. If a Sim walks through this pest breeding ground, there is a chance that the flies will swarm them and eat them alive. This event is more likely to occur if the Sim's hygiene is in the red.

The eaten Sim's ghost will be purple.
