Unlike in The Sims 2, it is unclear whose last name will be taken when they marry. Quite often the person being proposed to has to change their name, although this is not always the case. If the Sim whose name changed has children, they will also take on the last name of their step-parent.
If you wish for your couple to get married, you have to build up their relationship until they are in love, and have a romantic interest. Then after they have a full relationship bar, you will get the action: Propose going steady, under the romantic section. Your sims will then become boyfriend and girlfriend. When you then have a full relationship bar, you can propose marriage.
