Plastic Surgery is a mechanism that allows a Sim to change their facial and/or body structure. This feature appears in The Sims 2: University and The Sims 3: Pets.

The Sims 2: University

Sims who work in Show Business career will get Dr Vu's Automated Cosmetic Surgeon, a career reward that allows Sims to change their facial structure. When used, a change face interface a la CAS will appear, enabling the player to change Sim's face. The interface lacks undo button, so players cannot undo an action when choosing the face templates or changing the sliders. The effect of plastic surgery cannot be undone. Only young adults and older can use this object.

There's a chance that the machine will malfunction, causing Sims to have hideous face. Players can command Sims to take another surgery again afterwards, though. Plastic surgery is purely cosmetics; it won't change Sims' genetics. Thus, a child from a Sim who has used the machine will carry the Sim's original genetics.

The Sims 3

Sims can get a plastic surgery from the hospital. Sims can get either face or body surgery, to change their face structure, or weight, muscle mass, muscle definition, and breast size, respectively. There are cheap and expensive surgeries for each kind.
