These unborn twins are the youngest members of the Ottomas family, who shipped with The Sims 2: Seasons expansion pack. Not much is known about them, since their genders and personalities are randomly selected after their birth in-game.
Once the twins are born, the household will be at full capacity, since they live with their parents, three older siblings and paternal grandmother. Therefore, the option to try for or adopt a baby, or to invite another Sim to marry or move in, will not be available until the household is reduced by one - usually after the death of Dora, or when David goes to college or moves out.
There is a known glitch associated with the Ottomas twins, where the father in their family tree will be a random Sim instead of their supposed father, Peter. This has caused some players to believe that Samantha has been unfaithful (despite the fact that she is a Family Sim, and that it is her husband who aspires to Romance). However, since the "father" on the family tree is never consistent - and often observed to be biologically or chronologically impossible (e.g. a female Sim, a teen or child, a pet, an uncontrollable NPC, a long-deceased Sim or a Sim from another neighbourhood entirely) - it is more likely that this is simply a game glitch, which can be fixed by editing the twins' family trees in SimPE.
