The voodoo doll appears in The Sims: Livin' Large and as a reward for visiting a hidden lot in The Sims 2: Bon Voyage.
Bon Voyage
The doll can be used to do rather useless activities such as making a Sim hotter or colder or making their bladder motive go down. The doll almost always fails and backfires on the user frequently, making it almost completely useless and only fit for decoration or sales in Sim owned stores, if frequent visits are made to the Witch Doctor for new supplies.
Nonetheless, if a Sim wants the doll anyway despite all the pitfalls, they can acquire one from the witch doctor after repairing all of his broken appliances. This only relies on the Sim's mechanical skill to succeed. The witch doctor will lie in his hammock after being greeted, forever lying there until all of the appliances are repaired, and upon successful completion, he will will give the Sim the doll as a gift, however, he cancels the interaction frequently if the Sim is still busy and won't accept it, making it sometimes inconvenient to receive it as he'll only do it once per visit.
Five voodoo dolls may be collected on the souvenir shelf, nearby Sims will be cursed into imagining they are being attacked by a bee swarm.
