To initiate WooHoo, two adult sims need to have a high romance level with each other. Once that is completed, you can either look at the "Romance" interactions or click on a WooHoo spot. In the Sims 4, if caught WooHooing, both the people WooHooing and the person who saw will receive an "Embarassed" moodlet, the same moodlet as being walked in on while using the toilet or the shower.
