Alexandra Lothario, really just Lexie Lothario, is Don Lothario's lesser known half-sister. She is the daughter of Nicolo Lothario and Susan Lothario (née Wheeler). She works as an intern at Pleasantview Memorial Hospital, under her half-brother supervision.

After Nicolo and Adriana Lothario had Don, their couple went through a tough phase. Adriana was working all the time, leaving her son to her husband, who could not progress in his own career because of this. Adriana eventually had an affair with one of her colleagues, causing the couple to divorce. Nicolo moved to SimCity, where he met Susan Wheeler. They quickly got along and married the following year. Soon after, Susan gave birth to Lexie.

Lexie is a really quite and studious Sim. She showed great signs when she was only a child, which led her to go to SimCity private school. During her high school years, she made friends with Kendall Casablancas (née Walter). She was then accepted to Académie Le Tour where she majored in biology. Her father died during her sophomore year. He left a note saying he had already been married before, and had a son from his first marriage. After graduation, Lexie intended to follow SimCity General Hospital internship program, but changed to Pleasantview Memorial Hospital to get a chance to meet her half-brother. Her mother died a few days after she graduated, reinforcing her desire to move to Pleasantview.

The night before starting work, she went to a bar where she met Don Lothario first wife. Lexie asked her about Don, and she learned he had been married twice and had two children with his second wife. The next day, Lexie arrived at Pleasantview Memorial Hospital as an intern, and was assigned to Don supervision. She quickly told him she was his half-sister, causing him to reject her, as he did not accept her. Their relationship has been tensed ever since.

• She is loosely based on Lexie Grey, a character from ABC's medical drama Grey's Anatomy.
